limit hold'em assumptions

In establishing some parameters by which to study limit hold'em, we will make assumptions that may not always hold true but for now for our simple approach, we assume will always be true. Later as we become more advanced we may revisit these assumptions and perhaps challenge them.

For example, Howard Lederer's DVD on limit hold'em suggests that we always enter the pot with a raise as opposed to limping (just calling). We take this as gospel although we understand there may be some times when this is not true.
